Have a medical exam
The first step of the Connecticut Medical Marijuana Patient Registration Process is to get certified by a physician or APRN. This can be done quickly & easily through one of our telemedicine appointments!

Register
You must create an account with the DAS Business Network to access the online certification system.

Login
Login to your account using the email address and password you created during the registration process. Select 'I am a Patient' from the options menu and follow the system prompts to identify yourself.

Required Documentation
You must either upload or mail one (1) of the following:
• Proof of Identity (Drivers License, Passport, etc.)
• Proof of Residency
• Photo 1630 Certifications: Patients must read and affirm each statement on the webpage by selecting “Yes”.

As of July 1, 2023 - No more state fee!
The patient application fee WAS $100.00. Now it is $0! After you submit everything through biznet (in step 4), you will receive your new medical card in a separate email. It usually comes within 2 days.
